model monthly report format for local branches



dear presidents& secretaries of local branches,

kindly send monthly local branch activity report to the state IMA office, by filling the above application which will maximise on clicking and then take that print.
and make it a habbit to send all your branch members contact mobile numbers once every year to the state IMA office.
